If you have been seen at our office within the last 3 years, please do NOT schedule an appointment here. Click the link on our home page for EXISTING PATIENTS.

We are not in-network with the following insurances: Kentucky Passport through Molina Healthcare, Cigna Healthcare or any subsidiary of Cigna, MultiPlan (MPI), Beech Street, PHCS, PHCS Savility, Health EOS, ValuePoint, Prime Health Service, Integrated Health Plan (IHP), First Health, Encore, and Galaxy Health Network.

I'd Like to choose my doctor

New patients will generally see all three of our doctors during the course of treatment. Our online scheduling system does not offer scheduling with any specific doctor. If there is a certain provider that you would like to see for your initial visit, then you will need to contact our office directly at (502) 231-4003 for scheduling. Please keep in mind that once you are established with our office, you can schedule with whatever provider you choose.

Out-Of-Network Insurance
and Self-pay

If you have one of the aforementioned insurance plans and would still like to schedule with us, all treatment is charged as self-pay and must be paid out of pocket at the time of service. If you are not sure if we are an in-network provider for your insurance plan, please contact us by text or phone at (502) 231-4003 or by email using the link below.

Return & Maintenance Patients
Self-scheduling for existing patients is only for MAINTENANCE CARE. These visits will not be filed to insurance for any reason and require payment at the time of service. If you need to schedule an appointment for treatment that you expect to be covered by your insurance, please contact our office by phone or text at (502) 231-4003.
The Scheduling App does not give an option to choose which doctor you schedule with. If you wish to see a specific doctor, you must call or text our office at (502) 231-4003 to schedule. As a courtesy to our other scheduled patients, we can not accommodate a request to see a different doctor upon arrival when the appointment was made using the app. For MAINTENANCE appointment scheduling, download “The Scheduling App” in the iOS App Store or Android App Store or click the online button to be redirected to the desktop website.
First time users of the app, the Clinic ID to connect to our office is JNQKCB. You must enter the cell phone number and email address that we have on file in our system in order to access the schedule. We must have both on file in order for you to self-schedule.
